Registrations for tours are due no later than September 28, 2018 at 5:00 PM Eastern! Please return the completed form to the CTDA Office.

  • Complete all forms appropriate to you, using only one form for yourself and your companion. Incomplete registration forms will NOT be processed. You must completely fill out both pages of the form.
  • Fees must be paid in U.S. dollars, with no discounts given for attending only part of the meeting.
  • It is important that you indicate on the second page all functions you and/or your companion will attend, including optional events.
  • Special note for other family members attending : ALL attendees MUST have a meeting badge to be admitted to any meal event regardless of age. In order to prepare seating for all attendees and enable the hotel to provide adequate food and beverage, this will be strictly enforced.
  • Issue one payment covering all registration fees and optional events and be sure to include the payment with your registration form.

A Table Top Reception will be held on Monday, October 29th from 4:30-7:30pm. Any registrant ONLY attending the Table Tops and Reception qualifies for a free registration.

Pre-Conference Attendee

This registration will allow registrants to attend ONLY non-conference sessions being held October 27-28. You may attend the Opening Reception and either of the leisure events on Sunday, October 28th. If you will be attending the Monday-Tuesday educational sessions and meals you must fill out the full conference registration form. Note: In order to take advantage of this registration type you must submit a hard copy registration. You can download one at the top of this page.

Cancellations
Cancellations of conference registrations are acceptable upon written notice. Refunds cannot be guaranteed after September 28, 2018.
